11.2. Update Email Account Name

1. Fill out the Name & Address Change form on the CUW website.

2. Once the form has been sent in, email the IT Help-Desk at Support@cuw.edu with the following information:

• Your full name
• Your Student ID number (or username if not a student)
• Your email and phone number to be reached at
• The old name you are changing from as well as the new name you are changing to.

3. Wait for an email for phone call from IT with an update about your email name change.
